Advocacy and Hope: Housing and Disability Program Delivers Stability Adult Services Division

Riverside County’s Housing and Disabil ity Advocacy Program (HDAP) provides essential housing navigation and bene fits advocacy for adults with disabilities who are experiencing homelessness or are at risk of losing housing. HDAP staff deliver intensive case management, connect clients to shelter and rent al assistance, and guide them through complex disability benefits processes to support long term stability
Tracey’s experience reflects the pro gram’s impact. After leaving an abusive marriage and losing her job due to a fi bromyalgia diagnosis, she faced home lessness and was referred to HDAP for stabilization. The team secured tem porary housing, provided wraparound case management and supported her through multiple disability benefits de nials. With continued advocacy and per sistence, she completed medical doc umentation, pursued appeals and was
approved for disability benefits in Octo ber 2024, allowing her to begin planning for long-term stability. During Fiscal Year 2024/25, HDAP strengthened collaboration with Adult Services’ field teams, improved refer rals, expedited benefits applications, hired a dedicated social worker to ad dress caseload growth, and used avail able funds to maintain subsidies and ad dress increased rental costs.
