The Foundation on Aging of Riverside County is a 501(c)(3) charitable organization, established in 2004 as a catalyst for public-private sector partnerships to leverage existing resources, develop new and innovative solutions to meet the needs of older adults in Riverside County especially those that are underserved and vulnerable. The Foundation raises funds to fill gaps in funding for programs and services provided by the Riverside County Office on Aging, and by other community partners, that help sustain the quality of life of an ever-growing older adult population The Riverside County Office on Aging is charged with developing a network of care that offers services for vulnerable older persons and adults with disabilities. OoA has over 25 different programs aimed at promoting and supporting a life of dignity, well-being, and independence, including caregiver support, case management, and assistance with nutrition, transportation, and hospital/clinic post-discharge services. “Aging in Motion” (AIM) is Riverside County’s own approach to aging that emphasizes staying active, engaged, and dynamic as one grows older. RivCo-ONE’s AIM is a united community focused on a lifestyle of continuous movement and vitality. The Wellness & Recovery Program for Mature Adults is a comprehensive/integrated and consumer-driven behavioral health program to assist individuals with achieving their optimal level of functioning in the community and to enable older adult consumers to remain independent for as long as possible. A multidisciplinary behavioral health team approach is utilized in evaluating and addressing severe mental illness and issues of aging. Our goal is to prevent hospitalization, incarceration and homelessness and to promote healthy lifestyles during the recovery process. We provide behavioral health assessment/evaluation, peer support, family advocacy, therapy, psychiatry, field-based outreach, co-occurring substance abuse counseling, linkage to resources such as housing and primary medical care, benefits assistance, transportation, and after hours crisis support.
